Nativity School Off Campus Lunch
Permission Form

Throughout its history students at Nativity were permitted to leave campus at lunch time to go home or to patronize one of the restaurants in the Pleasant Ridge business district.  My son/daughter has my permission to leave campus during lunch.  I understand that the Nativity School Off Campus Lunch Permission Form must be signed and turned into the office every time.
