How to Apply
Request Information
This lets us know you are interested in Nativity and allows us to remind you when key events and dates are coming up via email.
Apply to Nativity
Starting February 1 the application window will open. Check back on this page for additional information and the link to the application. Families are encouraged to submit completed packets promptly as enrollment is filled on a first-come, first-served basis. All applications must be received by May 15.
You will need to enter the following as part of the application:
- Biographical information
- The $100 nonrefundable application fee, paid by credit card
- The email address of the current teacher (1st-8th grade only) for a recommendation
You will need to upload the following items to the application:
- Birth Certificate
- Baptismal Certificate (Catholic students only)
- Most recent report cards (1st-8th grade only)
- Request for Release/Transfer of School Records form (1st-8th grade only)
- Most recent Standardized Test Scores (2nd-8th grade only)
- Any existing Accommodations or Service Plans (if applicable)
The application deadline for the upcoming school year is May 15.
Round Up & Application Review
Incoming Kindergarten and 1st grade students will be invited to attend a Round-Up session in March where we work with the children individually and in small groups to assess their readiness and get to know the youngest prospective members of the Nativity community.
Students who apply for admission for grades 2 through 8 are assessed through a thorough review of the educational records and information we request. Because our resources for extra services are limited, we may need to have a prearranged conversation with parents in order for us to determine if we can meet your child’s academic needs.
Acceptance Letters/Meetings
Within two weeks of Entrance Testing or Kindergarten Round Up you will receive an Acceptance Letter with the results of the testing, or receive a phone call to set up a time to discuss your child’s needs. A meeting with the teachers and/or the principal may be needed to gather more information about your child or to provide you with more information about the programs and services available at Nativity.
EdChoice Application
Ed Choice Applications will be sent to families who may be eligible to apply for the EdChoice Scholarship from the Ohio Department of Education. Due dates are found on the forms.
Financial Aid Application
Parish members may submit a request for parish financial aid via the FACTS program when you complete the online enrollment process for your child.
Enrollment Forms
Once your child is accepted for Admission you will be asked to complete some additional forms online. These forms must be completed by their due dates in order for your child to start school.
Stay Up-to-date with Nativity
Contact us to be added to our mailing lists for the parish and school related to funerals, Holy Days of Obligation, important parish events, and the weekly Nativity News which includes information from the school and the parish bulletin.
